DescriptionPay per visit, flexible schedule.
Job
Responsibilities:
A.
Participate in the decision to recertify and discharge patients to/from Hospice.
Visit patients in home to assess recertification appropriateness for hospice and makeB.
treatment recommendations.
Will write treatment orders as appropriate.
C.
Work with the interdisciplinary care team to assure that the medical needs of the client are met.
D.
Documentation of recertification of a patients eligibility in the Hospice program.
E.
Work in cooperation with the attending physician to develop and update the plan of care.
F.
Serve as a resource to hospice staff, patients, families and attending physicians regarding pain and symptom control measure.
G.
Attend IDT meetings as required.
H.
Be available to interdisciplinary team members who have questions regarding the prognosis or care of Hospice patients.
I.
Provide educational in-services to physicians, facilities and community at large.
J.
Make routine home nursing visits as needed.
K.
Periodic chart reviews.
L.
Performs patient/family teaching; utilizes teaching tools and handouts; documents all teaching and patient responseAdditional Skills:
? Communicate effectively orally and in writing.
? Assess patients prognosis with regard to recertification of terminal illness.
? Provide leadership and guidance to the Hospice team.
? Job requires operation of an automobile and the ability to enter and exit automobile.
? To represent Hospice philosophy of care to the medical community.
? Participate in educational seminars and programs as appropriate to the overall educational needs of the Karen Ann Quinlan Hospice.

Board-licensed nurse practitioner, licensed to practice inNJ and PA.
Training and
Experience:
Experienced in the practice of family practice, oncology or hospital practice.
Job Knowledge:
Knowledge of the hospice philosophy of care and the delivery of medical care at home.
